Signify Health now available to small group metallic members
Arkansas Blue Cross and Health Advantage small group metallic members can now receive in-home evaluations through Signify Health. With Signify Health, a physician, nurse practitioner or physician assistant visits the member at home to conduct an in-depth evaluation of their medical, behavioral and social care needs. Signify Health has been available to individual metallic members for four years.
What is an in-home evaluation?
The in-home evaluation (IHE) is designed to close gaps in the member’s care and identify any non-medical needs they may have. The IHE includes the following:
- A comprehensive health exam and medication review
- An assessment of current and prior health conditions
- Screening for mental health conditions, such as anxiety, depression and suicidality
- Identifying safety hazards in the home, like fall risks, poor lighting and unsafe bathing conditions
- A clinical checklist recapping the visit and results
A member can have one evaluation per calendar year and the evaluation can be conducted virtually, if requested.
How are members identified for IHE?
Members are identified based on their claims data. Signify Health then attempts to contact the member to set up the IHE. Members can also request an IHE at signifyhealth.com.
How much does an IHE with Signify Health cost?
The IHE is provided at no cost to the member or the group.
What if the member already has a primary care provider (PCP)?
Signify Health is not a replacement for primary care. Signify Health sends a summary of the visit to the member’s primary care team to ensure coordination of care. Additionally, Signify Health can gain insight into social determinants of health (SDoH) issues not visible to a PCP, such as living conditions, travel issues and pharmacy access and share that information with the PCP.
If the member does not have a PCP or needs to see a specialist, Signify can connect members with those providers.
Who is eligible for an IHE?
Small group metallic members and their dependents 19 years of age and older are eligible. Only Arkansas residents are eligible.
